The Federal Family and Medical Leave Act (FMLA) provides eligible employees the opportunity to take 

unpaid, job-protected leave for certain medical and nonmedical needs for themselves and family 

members. The purpose of this policy is to provide employees with a general description of their FMLA 

rights. In the event of any conflict between this Policy and the applicable law, the employee will be 

afforded all rights required by law.


All Active Employees of the Company are eligible for the benefits under this policy provided they meet 

the following criteria as of the date the leave is requested:


• Have worked at least 12 (twelve) months with the Company.

• Have worked at least 1,250 hours for the Company over the 12 (twelve) months preceding the

date the leave would commence. 

• Currently work at a location where there are at least 50 (fifty) employees within 75 (seventy-five) miles.
