LG offers Medical Coverage through Horizon Blue Cross Blue Shield of New Jersey at no cost to you and your eligible Dependents. Premiums are 100% company-paid. When you enroll in Medical, you and your eligible Dependents will automatically receive Vision and Prescription Coverage. This is called a Bundled Benefit and there is only one Benefits Card. 

This card is also used for Medical, Vision and Prescriptions (RX). If you have not received your Insurance Card from Horizon, please contact Horizon Blue Cross Blue Shield at (800) 355-2583 www.horizonblue.com/lgelectronics. You may also request that a card be mailed to your Home Address. 

When you call, be sure to also request your Member ID Number. You will need this information to Register and log in to the Horizon Member Portal, where you can download and print a temporary insurance card while you wait for your permanent card to arrive by mail.
