If you're Enrolled in the LG Medical Plan, you will have access to HorizonbFit℠ which is an incentive program that pays you when you achieve your personal fitness goals. You can earn up to $240 a year in Rewards! When you enroll in the Program, you become eligible to receive a $20 reward each month in which you complete 12 or more Wellness Activities.
An approved Wellness Activity is visiting any of the nation’s 30,000 participating facilities such as Gyms, Yoga and Pilates Studios, Physical Therapy Centers, Community Centers and more.
HOW TO ENROLL: To enroll, visit www.horizonbfit.com. Verify your eligibility using your BCBS Member ID Card number and set up your account. If you have dependents 18 years or older covered on the Medical Plan, they are also eligible to participate. Download the ActiveFit™ App to help track your activities. The App can detect when you’ve entered a participating facility and log the visit toward your Monthly Incentive. If the facility you visit is part of the Advanta Fitness Network, you can nominate it for participation through the Member Portal!
