Please see the information below to update a Name Change in LG4Me. 

• In order to complete your Name Change, please email one (1) of the following pieces of supporting documentation showing your name as requested:

   

  • Social Security card showing your new name as you are requesting it

  •  State Driver’s License 

  • US Passport

   

  We will finalize your request upon receipt of the one of the items requested above. Please be sure to access LG4Me and submit your Name Change Request.
